The information below will help us find your details and close the referral we have on the waiting list. We need you to fill in the details where it says "required". If the Legal Surname and Referred Surname are the same, please enter them both twice.

After you fill out the form and click the green confirm button, you will get a confirmation email within 5 working days.

Once we have taken care of your request, we will email you to let you know that the referral has been closed. If it turns out that the referral was closed by mistake, we can reopen it and set the original referral date.
